Prior to your initial treatment, a form will be emailed over to you. This will enable me to ascertain current lifestyle, health information and what you are hoping to gain from your treatment. This will then be discussed at your first treatment. 

 

All data gathered will be handled with the strictest of confidence and be stored according to the Data Protection Act. You have a right to access your details and they are reviewed and amended when necessary.
